What is Breast Revision?

Breast revision surgery is a secondary procedure performed to reshape the breasts and remove or replace implants from a prior breast augmentation. More complex than a first-time augmentation, revision requires advanced surgical techniques and an elevated level of expertise to address issues such as implant malposition, size change, rippling, capsular contracture, or simple dissatisfaction with appearance. Some women opt to exchange their implants for a different size or material, while others prefer breast explant surgery to return to their natural size. When implants are removed without replacement, a breast lift can tighten skin that was stretched by the original implants.

Types of Breast Revision

Replaces existing implants with larger or smaller devices. Ideal for patients whose size preferences have evolved. Involves removing the original implants, adjusting the pocket as needed, and inserting new implants for balanced proportions.

Switches saline to silicone (or vice versa) to improve feel, weight, or peace of mind. Pocket reinforcement or ADM support may be added for stability.

Removes thickened scar tissue (capsular contracture) or tightens a loose pocket. Restores softness and positions the implant correctly within the breast.

Removes implants completely and performs a mastopexy to reshape and tighten natural tissue. Perfect for women wishing to return to their native size while maintaining upper-pole fullness.

Uses biologic or synthetic mesh to reinforce weakened tissues, enhancing implant support and preventing future displacement. Suited to complex or recurrent revisions.

Transfers your own purified fat to soften edges, conceal rippling, or add subtle volume where needed. Often combined with implant exchange for the most natural feel.

Relocates implant position above or below the muscle to improve contour, reduce animation deformity, or address thinning tissues.

At South Coast Plastic Surgery, planning begins with understanding why revision is needed, whether due to cosmetic concerns, implant-related complications, changes in preference, or medical reasons. When you meet with your surgeon, you’ll discuss:

  • Your original breast surgery and implant details
  • Current concerns such as firmness, asymmetry, discomfort, or appearance
  • Whether implants should be exchanged, repositioned, or removed
  • Implant size, type, and placement options
  • The possible need for a breast lift to restore shape and balance
  • Recovery expectations and activity restrictions
  • Long-term goals and preferences

Your surgeon will also assess the condition of the implant capsule, implant position (over or under the muscle), nipple position, and skin elasticity. Whenever possible, previous incisions are reused to limit additional scarring and help preserve sensation.

Your Breast Revision Journey and Recovery

From your first consultation to your final results, you’re guided through every stage of your Breast Revision journey with personalized support, automated instructions, and 24/7 access to your care team. This is your journey, we just make it easier.

Comprehensive Consultation & Assessment

Your journey begins with a thorough consultation and review of your surgical history. Imaging or operative records may be reviewed when available. A personalized plan is created to correct the underlying issue—whether that involves implant exchange, capsule adjustment, repositioning, or implant removal.

In some cases, breast revision is combined with a breast lift to restore shape and symmetry after implants are corrected or removed.

The Breast Revision Procedure

The complexity of implant revision surgery varies from patient to patient, according to the reasons for pursuing the procedure. Some patients will require more complicated techniques, as in the case of capsular contracture or repositioning the implants from sub-glandular to sub-pectoral. A breast lift may also be performed in tandem with implant revision to achieve a balanced and aesthetically pleasing final result.

Surgery is performed under light general anesthesia. Whenever possible, our team will aim to reuse previous incisions so as not to create any new scarring. This helps to minimize recovery time as well and helps preserve sensation in the nipples.

Your procedure will include the following:

  • Exchanging the implants for a new type, size, or shape
  • Adding saline to mitigate rippling
  • Adjusting the implant capsule to address implant placement issues

Breast implant revision typically takes 2-4 hours to complete. This is near twice the time needed for the original augmentation surgery since revisions are more technically challenging for the surgeon. However, revision augmentation usually comes with less recovery time and is associated with less pain than the first surgery since no new pocket needs to be created from scratch.

Breast Revision Recovery Timeline

Recovery varies depending on the extent of correction performed.

Immediately after surgery

  • Swelling, bruising, and chest tightness are common
  • Discomfort is typically less than the initial augmentation
  • A supportive surgical bra is worn

First week

  • Most patients return to work and light daily activities
  • Swelling and soreness gradually improve

Weeks 2–4

  • Strenuous exercise and heavy lifting are restricted
  • Gradual return to normal activity once cleared

Some revisions—such as implant repositioning—may require slightly longer recovery, but overall downtime is often shorter than the first surgery.

Breast Revision Results

Results are noticeable early, though final outcomes continue to refine as swelling resolves and tissues settle. The goal of revision surgery is correction—addressing the underlying problem while restoring balance, comfort, and a natural appearance.

You can expect to resume light activities within 5-7 days and can return to work within 7-10 days. Most patients follow this timeline:

Complete recovery and healing take approximately 3-6 months. During this time:
  • Month 2 - 3

  • Month 3 - 6:

  • Long Term

What kind of implants do we offer?

Plastic surgeons in the U.S. now have four main choices of manufacturers of silicone and saline breast implants with FDA approval. At South Coast Plastic Surgery we offer: Motiva, Sientra, Allergan, and Mentor, providing you with the broadest range of high-quality options to achieve your ideal outcome.

Motiva Silicone Gel Implants

Motiva® Round and Ergonomix® implants are now FDA-approved for augmentation (primary and revision). These implants feature the patented SmoothSilk® surface designed to enhance biocompatibility and lower inflammation. They also include an ergonomic gel shell that helps the implant adapt to body position. Clinical trial data show very low complication rates: e.g., less than 1% for capsular contracture or rupture.

At South Coast Plastic Surgery, we now recommend Motiva as our first line option for patients seeking advanced silicone gel implants with the latest safety and ergonomic design.

Sientra Silicone Gel Implants

Sientra silicone gel breast implants are available only to board-certified plastic surgeons and are not available to MD’s who have not gone through the rigorous training to achieve board-certification. Sientra implants are produced with the latest in silicone gel technology, creating an implant that is formable.

These implants have the advantage of retaining their shape whether you are standing up or lying down, which prevents them from rippling or folding in their shells. Sientra implants have an extremely low rupture rate, which reduces the risk of the need for a secondary surgery due to a ruptured implant.

Mentor Cohesive Gel Implants

We also offer Mentor cohesive gel implants, known for their natural feel and proven safety record. Mentor implants provide excellent shape retention and are a popular choice among patients seeking balanced, natural-looking results.

Allergan Semi-Solid Implants

Allergan semi-solid implants offer another premium option, providing a soft, natural feel with excellent durability. These implants are designed to maintain their shape while moving naturally with your body.

Is saline or silicone best?

For women younger than 22 years of age, saline implants are the only commercially available option for cosmetic breast augmentation. Some women opt for saline because they are more comfortable with the idea of an implant made with a saltwater solution. Saline implants are not filled until after insertion, so the incisions can be slightly smaller than those needed when placing silicone implants.

What’s right for you?

The majority of women prefer silicone implants as they tend to produce a more natural look and feel. Today’s silicone gel is semi-firm to the touch yet is still malleable and pliant, and closely mimics the bounce and feel of natural breast tissue.

What are anatomical breast implants?

In breast augmentation, the shape of the implant plays a key role in determining your final breast appearance.

Teardrop-Shaped Implants

Anatomical implants are shaped like a natural breast, with more volume concentrated at the bottom and tapering off toward the top. These teardrop-shaped implants are more popular for women with little natural breast tissue in performing breast reconstruction surgery. Anatomical implants are also the most common implant shape of choice for older women, as the graduated shape more closely resembles the natural appearance of a breast at that stage of life.

What are round breast implants?

Round Breast Implants

Round implants provide volume throughout the breast, including the upper portion of the breast. While some women seek this aesthetic, as it gives a full, high look, others believe that this upper volume makes the implants look too unnatural.

During your consultation, Dr. Song or Dr. Benyamein will help you determine which implant shape best suits your anatomy and aesthetic goals.

Breast Revision Results and Safety Considerations

Breast revision surgery addresses both cosmetic and medical concerns. Understanding how technique, healing, and prior surgery affect outcomes can help set realistic expectations.

Achieving Natural-Looking Results

Natural-looking breast revision results depend on accurately identifying the cause of the issue and choosing the appropriate corrective technique. Revision may involve changing implant type or size, correcting implant position, softening hardened scar tissue, or reshaping the breast with or without a lift.

At South Coast Plastic Surgery, revision planning focuses on restoring proportion and symmetry while preserving breast tissue and sensation whenever possible. When performed thoughtfully, breast revision can significantly improve comfort and appearance without drawing attention to prior surgery.

Is Scarring a Concern?

One advantage of breast revision surgery is that existing incisions can often be reused. This helps:

  • Avoid creating new scars
  • Limit additional tissue disruption
  • Support preservation of nipple sensation

If new incisions are required, they are planned carefully to follow natural contours and minimize visibility.

Scar Healing Timeline

Scar healing usually follows a gradual progression:

  • Weeks 1–2: Redness and mild firmness
  • Months 1–3: Softening and flattening
  • 6–12 months: Continued fading and refinement

Proper scar care and follow-up support optimal healing.

Understanding Risks and Safety

Breast revision surgery is safe when performed by an experienced plastic surgeon, but it carries risks due to the presence of implants and scar tissue.

Potential risks include:

  • Infection or bleeding
  • Capsular contracture recurrence
  • Implant rupture or malposition
  • Changes in breast or nipple sensation
  • Asymmetry or need for further revision

Risks vary depending on the reason for revision and surgical complexity.

Reducing Your Risk

Risk reduction is supported by:

  • Thorough evaluation of prior surgery and implant history
  • Careful surgical planning and technique
  • Reuse of prior incisions when possible
  • Surgery performed in an accredited facility
  • Clear recovery guidelines and follow-up care

With proper planning and experienced care, most patients experience meaningful improvement and long-term satisfaction after breast revision.

Dr. Song performed my breast implant surgery nearly 10 yrs ago with the ideal breast implant. I've had no issues in this time. I live on the other side of the country now, but I would've loved to come back for removal, since I'm on an all natural journey. Frequently Asked Questions About Breast Revision

Everything you need to know about Breast Revision, explained by South Coast Plastic Surgery's experts

About the procedure

How is breast revision surgery different from my first augmentation?
Revision often involves correcting scar tissue, adjusting the implant pocket, or combining a lift, all of which add complexity. Your surgeon tailors the plan to address existing implants and tissue changes.
Will my old scar be used?
Whenever feasible, we reopen the original incision to minimize additional scarring. If a new incision is needed, we place it in a discreet location.
Can I change from saline to silicone implants?
Yes. Many women choose to upgrade to cohesive silicone gel implants for a softer feel and more natural movement.
What if I want my implants removed permanently?

A breast explant with lift can remove implants, tighten tissue, and reshape the breast.

• Explant only: removes implant and capsule

• Explant + Lift: removes implant and lifts tissue for improved contour

Recovery & Results

How long is recovery after revision surgery?
Most women return to desk work within 7-10 days and resume full exercise by 6 weeks. Swelling can take several months to completely resolve.
Will I need drains?
Drains are occasionally used for extensive capsule removal or seroma risk. If placed, they are usually removed within the first week.
When will I see my final results?
You will notice an immediate change, but final position and softness develop over 3-6 months as tissues settle.
Is the procedure painful?
Discomfort is moderate and well managed with oral pain medication and muscle relaxants. Most patients report the recovery is easier than their first augmentation.

Safety & Risks

Can I breastfeed after revision?
Many women are able to breastfeed after revision, particularly when the surgery preserves glandular tissue. Discuss your plans with your surgeon.
What is capsular contracture and can it return?
Capsular contracture is a tightening of scar tissue around the implant that can distort shape. Revision with total or partial capsulectomy reduces the risk, but recurrence is still possible.
How do you minimize infection risk?
We employ a strict no-touch technique, antibiotic irrigation, and a sterile environment to keep infection rates exceptionally low.
Are mesh or ADM safe to use?
Biologic and synthetic meshes have been thoroughly studied and are considered safe tools to provide additional support when tissues are weak.

Cost & Insurance

Will insurance cover breast revision?
Most revisions are cosmetic and not covered by insurance. However, a portion may be covered if the surgery addresses implant rupture or certain complications.
What factors influence the cost of my revision?
Complexity of the procedure, need for mesh, implant choice, and operating time all affect pricing. A detailed quote is provided after your consultation.
Do you offer financing?
Yes. We partner with CareCredit and Cherry to provide convenient monthly payment plans.

Lifestyle & Long-Term Care

Will I need another surgery in the future?
Implants are not lifetime devices. Future surgery may be desired for cosmetic reasons or implant maintenance, but many patients enjoy their results for 10-15+ years.
How can I maintain my results?
Maintain a stable weight, wear supportive bras, and follow regular follow-up appointments to keep your breasts looking their best.
How often should implants be checked?

We recommend annual exams and an ultrasound or MRI every few years to ensure implants remain intact and healthy.
