Breast reconstruction isn’t one single procedure. It’s a set of options that can restore breast shape after mastectomy (removal of the breast) or help reshape the breast after lumpectomy (removal of part of the breast). Most reconstruction falls into two main categories: implant-based reconstruction or flap (your own tissue) reconstruction.
During your consultation, your surgeon will help you understand which approach fits your anatomy, cancer treatment plan, and personal goals, including whether reconstruction is best done immediately (at the time of mastectomy) or delayed (after other treatments).
Implant-Based Breast Reconstruction
Implant-based reconstruction rebuilds the breast mound using a saline or silicone breast implant. We will work with your cancer surgeon in planning your reconstruction. When possible, it will be performed immediately after your mastectomy. This is called an “immediate reconstruction”.
- Direct-to-implant (selected cases): In some patients, the implant may be placed at the time of mastectomy, depending on skin quality, surgical plan, and overall treatment needs.
In other cases, a staged approach will be required. A tissue expander may be needed to stretch the skin to accommodate an implant. The expander has a port through which saline solution is added over time. This may require several months.
- Tissue expander → implant (staged reconstruction): A tissue expander (a temporary device) is placed first and gradually filled over time. Later, it’s exchanged for a permanent implant.
Flap Breast Reconstruction (Using Your Own Tissue)
Flap reconstruction uses tissue from another area of your body to form the breast mound. This tissue may come from the abdomen or other donor areas depending on what’s safest and appropriate for you.
Because flap reconstruction involves both the breast and a donor site, recovery is typically longer than implant-only reconstruction. Many patients value that the reconstructed breast is made from their own tissue, which can feel soft and natural.
The last step may be to reconstruct a natural-looking nipple. The skin is formed into a nipple shape with sutures, and as the final step, it will be tattooed to create the appearance of a nipple. The result is remarkably natural in appearance.
